The Central East Stroke Network (CESN) Integrated Stroke Program Model: Vision was developed as part of a comprehensive stroke system redesign initiative (2012-2014). It has been reviewed and updated to reflect emerging best practices. This model represents a high-level overview of CESN’s vision for an integrated system of stroke care.
ESD is defined in the Quality Based Procedures (QBP): Clinical Handbook for Stroke (p. 87) as “a form of rehabilitation designed to accelerate the transition from hospital to home through the provision of rehabilitation therapies delivered by an interprofessional team in the community. It is intended as an alternative to a complete course of inpatient rehabilitation …”.
