CESN Integrated Stroke Program Model: Vision

The Central East Stroke Network (CESN) Integrated Stroke Program Model: Vision was developed as part of a comprehensive stroke system redesign initiative (2012-2014). It has been reviewed and updated to reflect emerging best practices. This model represents a high-level overview of CESN’s vision for an integrated system of stroke care.

CorHealth Stroke Quality Based Procedures Resource Centre Provides health system planners and clinicians with tools and resources to support implementation of Stroke Quality Based Procedures across the care continuum

Stroke Distinction Accreditation A specialized quality improvement program based on the Canadian Stroke Best Practice Recommendations for organizations with a dedicated stroke services program or unit and networks providing acute and/or inpatient rehabilitation

Secondary Stroke Prevention To support a secondary stroke prevention clinic model of care the following three deliverables were produced: • Core Elements recommended for SPCs • Quality Indicators • Implementation Toolkit A Stroke Prevention Infographic for Primary Care Providers, Standardized Secondary Stroke Prevention Referral Criteria and Patient Summary were subsequently developed to help operationalize core elements. These documents are modifiable to reflect regional and local settings. Early Supported Discharge (ESD)

ESD is defined in the Quality Based Procedures (QBP): Clinical Handbook for Stroke (p. 87) as “a form of rehabilitation designed to accelerate the transition from hospital to home through the provision of rehabilitation therapies delivered by an interprofessional team in the community. It is intended as an alternative to a complete course of inpatient rehabilitation …”.
