Mackenzie Health (MH) is the District Stroke Centre serving the District of York and the only stroke centre located in the Central LHIN.
In addition to the District Stroke Centre at MH, there are two community hospitals in the district of York: Markham Stouffville Hospital (Markham site) and Southlake Regional Health Centre.
Hyperacute:
Mackenzie Health (MH) is the tPA delivery site and EVT triage Centre for the district of York. All 911 calls addressing a potential stroke patient in the York region will be sent directly to MH for immediate assessment for stroke treatments.
Stroke Unit:
Integrated stroke units combine acute and rehabilitation beds on one specialized unit. In York region, there are a total of two integrated stroke units: Mackenzie Health, and Markham Stouffville Hospital (Markham site).
Rehabilitation (Inpatient & Outpatient):
Inpatient rehabilitation services are offered for stroke patients at all three hospitals in York region through the integrated stroke units described above, and through a mixed rehabilitation unit at Southlake Regional Health Centre.
Some patients may be suitable to attend a Cardiovascular Rehabilitation Program at various locations across our district. A physician/nurse practitioner referral is necessary to access these programs.
Stroke Prevention Clinic:
Mackenzie Health is the leader for Stroke Prevention Clinics affiliated with the Central LHINs Stroke Prevention Strategy. These clinics are located at Mackenzie Health Richmond Hill Hospital, Southlake Regional Health Centre, Markham Stouffville Hospital, Humber River Hospital, and North York General Hospital. Through this strategy, there are also links made to Cardiovascular Rehabilitation clinics across the district.
These Stroke Prevention Clinic are for people who have had signs and symptoms of a stroke that if untreated can lead to a stroke. It is also meant for those who have experienced a stroke and require ongoing care to prevent a second stroke.
Community:
Upon discharge from the hospitals in York region, every effort is made to provide a seamless transition into the community through comprehensive discharge planning. All eligible patients will be referred to the appropriate Stroke Prevention Clinic for follow-up and, if indicated, will initiate a referral to Cardiovascular Rehabilitation. Referrals are initiated for those patients requiring out-patient rehabilitation and /or other community services in collaboration with patients and families.
As a District Stroke Centre, Mackenzie Health supports and informs the planning of community services that address current gaps in stroke care across York Region and Central LHIN.
